Fat, skin or muscle: what needs to change?

Body contouring works on three things: fat, skin and the abdominal muscles, and which of them is the problem decides the operation. On her Turkish website, Op. Dr. Bilgehan İlker gives the thigh as an example: in some people liposuction alone is enough, in others loose skin has to be removed as well, and only an examination shows which.

Stubborn fat with firm skin:

liposuction removes fat through small incisions. Ultrasound-assisted liposuction uses ultrasound energy to loosen fat before it is removed, and HD liposuction aims to show the outline of the muscles in people who are already lean. Liposuction does not tighten loose skin, remove stretch marks or treat cellulite.

Loose skin and separated muscles in the abdomen:

a tummy tuck removes excess skin and fat from the lower abdomen and, in most cases, repairs the abdominal muscles. Stretch marks are removed only on the skin that is cut away.

Changes after pregnancy:

a mummy makeover combines procedures, typically a tummy tuck with a breast lift or augmentation, often with liposuction.

Loose skin after major weight loss:

an arm lift, thigh lift or body lift removes excess skin. These operations leave long scars, and where the scars will sit is part of the decision.

Intimate concerns:

labiaplasty reduces or reshapes the inner labia. A wide variation in labia size is normal, and NHS guidance is that the operation should not be done before 18, because the labia keep developing into early adulthood.

Weight, timing and health

  • Complication rates rise with body mass index (BMI). In a study of nearly 128,000 cosmetic surgery patients, they rose from 1.6% at a normal BMI to 3.1% at a BMI of 30–39.9 and 4.2% at 40 or above.
  • After pregnancy, surgery is usually postponed until at least six months after the birth, and longer if you are breastfeeding.
  • After weight-loss surgery, weight usually stabilises within 12 to 18 months. Skin-removal operations are planned after that, sometimes in stages, one area at a time.
  • If you take a GLP-1 medicine such as semaglutide or tirzepatide, mention it early. The anaesthetic team decides whether and when to pause it before surgery.
  • Nicotine slows healing: in one series of tummy tucks, wound-healing problems affected 47.9% of smokers and 14.8% of non-smokers. You will be asked to stop smoking and vaping at least four to six weeks before surgery.

Combining procedures and blood clots

Body operations carry a higher risk of blood clots (deep vein thrombosis and pulmonary embolism) than face or breast surgery, and the risk rises when procedures are combined and operating time is long. In a study of more than 25,000 tummy tucks, major complications occurred in 3.1% of operations done alone and in 10.4% when liposuction and other body contouring were added. Your personal clot risk is assessed before surgery, and preventive measures are planned accordingly.

Recovery and flying home

  • NHS guidance is to wait 5–7 days before flying after liposuction and 7–10 days after a tummy tuck.
  • After a Brazilian butt lift, long periods of sitting are avoided for about two weeks, which affects when and how you fly home.
  • Compression garments are usually worn for several weeks, day and night at first.
  • Swelling takes months to settle: the final contour after liposuction usually shows at three to six months.

Frequently asked questions

How much weight will I lose with liposuction?

Very little, and that is not its purpose. Liposuction reshapes areas of stubborn fat; it is not a treatment for obesity and does not replace diet and exercise, so there is no typical figure in kilograms. It works well when you are close to your ideal weight. A tummy tuck is not a weight-loss operation either: it removes skin and fat and repairs muscle.

Can the fat come back after liposuction?

The fat cells that are removed do not come back. If you gain weight, though, the remaining fat cells grow, and new fat can collect in other areas, including around the abdomen. Keeping your weight stable and exercising regularly protects the result: in one study, women who did not exercise after liposuction showed an increase in abdominal fat.

Should I lose weight before body contouring?

Ideally you are close to your target weight and have kept it stable for some months. Complication rates rise with BMI, and further weight loss after surgery can loosen the skin again. The guidance sets no single BMI limit, so Dr. İlker assesses your weight and health at examination. After weight-loss surgery, skin-removal operations usually wait until weight has been stable, often 12 to 18 months on.

Can I have a tummy tuck if I want more children?

You can, but most surgeons advise waiting. A later pregnancy stretches the abdominal skin and muscles again and can undo part of the result. ASPS advises women who are planning future pregnancies to postpone the operation. If you are not sure about having more children, raise it at your consultation so that the timing can be planned with you.
