Choosing by what you would like to change

A face rarely changes in one place only, so Op. Dr. Bilgehan İlker looks at proportion, skin quality and volume together before suggesting anything. A heavy upper eyelid, for example, is sometimes caused by a low brow, and treating the eyelid alone can make the brow look lower.

The nose.

Rhinoplasty reshapes the bone and cartilage of the nose. When only the tip needs work, a tip rhinoplasty may be enough. When breathing is also a problem, a septorhinoplasty straightens the septum in the same operation, and a nose that has been operated on before needs a revision rhinoplasty. Swelling settles slowly: the final shape can take around a year to show.

Eyes and brows.

Blepharoplasty removes or repositions excess skin and fat in the upper or lower eyelids, often under local anaesthetic with a same-day return home. A brow lift raises a low brow, and a fox eye lift lifts the outer corner of the eye and brow.

Ageing in the face and neck.

A facelift lifts the deeper tissues of the cheeks and jawline and removes loose skin, and a neck lift treats loose neck skin and muscle bands. Lost volume can be restored with your own fat through facial fat transfer.

Profile and contours.

Chin surgery changes the projection of the chin, and cheek augmentation adds volume over the cheekbones. Buccal fat removal slims the lower cheeks through small incisions inside the mouth and suits people at a stable weight. Fullness under the chin with firm skin may respond to chin liposuction.

Procedures that are often combined

Some operations treat neighbouring areas and fit well into one anaesthetic: a facelift with a neck lift, eyelid surgery or fat transfer, or rhinoplasty with chin surgery to balance the profile. Combining means one recovery instead of two, but a longer operation and a longer stay in Istanbul.

Recovery and travel after face surgery

  • Bruising and swelling are at their most visible in the first days. After a facelift or eyelid surgery, most people feel ready to be seen in public after 10–14 days.
  • ASPS, the American Society of Plastic Surgeons, advises waiting 7–10 days after facial procedures before flying.
  • As a guide, plan around a week in Istanbul for smaller operations under local anaesthetic, 7–10 days after surgery under general anaesthetic, and at least 10–14 days after long or combined operations. Your surgeon confirms your return date after your check-up.
  • Final results take time: around a year for the nose, and six to nine months after a facelift.
  • Protect healing scars from the sun for the first 12 months.

Frequently asked questions

Will there be visible scars after face surgery?

Some, but most incisions are placed where they are hard to see: in the natural crease of the upper eyelid, around the ear and into the hairline for a facelift, behind the ear for otoplasty, inside the mouth for buccal fat removal, and across the narrow strip between the nostrils in open rhinoplasty. Facelift scars usually fade over 12–18 months, although they can be harder to hide in men with short hair.

Can face surgery be done under local anaesthetic?

Some procedures, yes. Upper eyelid surgery, buccal fat removal, chin liposuction, lip lift and ear correction in adults are often done under local anaesthetic, sometimes with sedation, and you usually go home the same day. Rhinoplasty and full facelifts are normally done under general anaesthetic, often with one night in hospital. Your plan states which applies to you and how long to stay in Istanbul.

Is there an age limit for facial surgery?

For cosmetic surgery, Dr. İlker's policy, stated on her Turkish website, is to operate on adults aged 18 and over; for the nose, the exception is surgery after an injury. Ear correction is different: it can be done from about five years of age, once the cartilage is firm enough to hold its new shape. Beyond that, your general health matters more than your age.

How do I choose between a facelift and non-surgical tightening?

It depends mainly on how much loose skin there is. Ultrasound skin tightening and thread lifts suit early laxity and give a milder lift that fades with time. When skin hangs along the jawline or neck, surgery is what treats it. On her Turkish website Dr. İlker describes the same order: skin quality first, then injectables and non-surgical tightening, and a facelift for more advanced changes.
