What is ultrasound skin tightening?
Ultrasound skin tightening uses microfocused ultrasound to heat tiny points 4.5 mm and 3 mm beneath the skin without breaking the surface. The heat prompts new collagen, which gradually firms mild to moderate laxity of the brow, lower face, under the chin and neck. A single session takes about 30–90 minutes, with no recovery time. Results develop over 2–3 months, sometimes up to six, and improvement can continue for up to about a year. It is not a substitute for a facelift.

Techniques
Several devices tighten skin with heat, using different energy sources.
Microfocused ultrasound with imaging
Op. Dr. Bilgehan İlker's own pages describe this method, using the Ulthera system. An ultrasound image shows the layers under the skin, so energy reaches set depths. In the US it is cleared to lift the brow and the skin under the chin and on the neck, and to improve chest lines.

Radiofrequency heats the skin with electromagnetic energy rather than ultrasound. Changes appear over two to six months, and several sessions may be needed.

Op. Dr. Bilgehan İlker is a specialist in plastic, reconstructive and aesthetic surgery, trained at Dokuz Eylül University. Her own pages name microfocused ultrasound as her preferred method for non-surgical tightening of the face and neck, given in a single session.

Results and how long they last
The change comes as collagen forms; judge it at two to three months.
The effect is moderate tightening, not a surgical lift: in one study the brow rose by an average of 1.7 mm.
How long it lasts: Improvement can continue for up to about a year; ageing continues.

Frequently asked questions
How long do the results last?
Improvement can continue for up to about a year. Because skin keeps ageing, a further session may be considered later, and some people benefit from additional sessions. How long it lasts varies from person to person.
When will I see results?
A slight lift may show early, but the main change develops as new collagen forms: typically over 2–3 months, and sometimes up to six. Clinical studies assessed results at three months, so judge yours then rather than in the first weeks.
Does it hurt?
You feel heat and tingling as the energy is delivered, and intensity varies between people and areas. In one lower-face study, average pain during treatment was about 6 out of 10. Numbing cream or a local anaesthetic can be used if you wish.
Can it damage facial nerves?
Temporary nerve effects, such as numbness, tingling or muscle weakness, can occur and usually recover within 2–6 weeks. Clinical studies did not report permanent facial nerve damage. Imaging the tissue before treatment helps keep the energy at the intended depth.
Can it cause fat loss or a hollow face?
Fat or volume loss has been reported after treatment and is listed among the device's post-market adverse events. A case series also reported rare serious complications, including a dent caused by tissue loss after swelling. Correct depth and careful technique matter, and any new hollow should be reported.
Is it the same as HIFU?
Not exactly. HIFU, high-intensity focused ultrasound, is a broad term that also covers medical uses such as tumour treatment. The system on Op. Dr. Bilgehan İlker's pages adds imaging and has US clearance for specific uses; other cosmetic devices differ, so ask which is used.
Is this a non-surgical facelift?
The term is used in marketing, but no. Non-surgical methods do not give surgical results. Ultrasound tightening can firm mild to moderate laxity a little, whereas a facelift repositions deeper tissues and removes excess skin for a much more substantial change.
Can I have it after dermal fillers?
Yes, but the device instructions advise against delivering energy directly over areas containing filler. Tell Dr. İlker where and when you have had fillers, so treatments can be planned in the right order and the energy kept away from them.
Can I have it while pregnant?
It has not been evaluated in pregnancy or breastfeeding, so treatment is postponed. The same caution applies to some conditions, including autoimmune disease, diabetes, epilepsy and bleeding disorders. Tell us about your health in full before any treatment is planned.
How many sessions do I need, and does it suit a short trip?
Usually one session. As there is no recovery time, it fits a short stay in Istanbul: a consultation, the treatment and perhaps a day for tenderness to settle. The result develops at home, so agree beforehand how it will be reviewed.
