What is double chin liposuction?

Chin liposuction (submental liposuction) removes fat that collects between the chin and the neck, through openings a few millimetres long. It aims to sharpen the angle between jaw and neck and define the jawline. When the skin is elastic, it can often be done under local anaesthesia, and most people go home the same day. Swelling and bruising ease within about two weeks, and the area can take up to six months to settle. If the skin is loose, liposuction alone is not enough.

What it can do

  • Remove fat under the chin that diet and exercise have not shifted
  • Sharpen the angle between the chin and the neck
  • Give a clearer jawline in profile

What it cannot do

  • Tighten loose skin or neck bands; that needs a neck lift or chin tuck
  • Treat being overweight; it is not a weight-loss method
  • Correct fullness caused by a receding chin or large salivary glands

Is it right for you?

Skin elasticity, more than age, decides whether liposuction alone will work.

Usually a good fit if

  • You have a distinct pocket of fat under the chin
  • Your weight is normal or close to normal, and stable
  • Your skin is elastic enough to tighten over the new contour
  • You are healthy, do not smoke and have realistic expectations

Better to wait or consider alternatives if

  • The skin of your neck is clearly loose, or there are visible muscle bands
  • You are overweight overall or plan to lose a lot of weight
  • You have a bleeding disorder or a health problem that is not under control
  • Worry about your appearance is causing real distress; talk it through before any surgery
  • You are under 18

Techniques

Most double chins are treated with tumescent liposuction. Loose skin needs a tightening operation instead.

Tumescent liposuction

Fluid containing local anaesthetic and a medicine that narrows blood vessels is injected, then fat is removed in even layers through a fine cannula. Laser or ultrasound energy is sometimes used to loosen the fat first.

Best for

Most people with elastic skin.

Chin tuck or neck lift

When skin or muscle is lax, excess skin is removed and the neck muscle tightened through an incision under the chin, or around the ears in a neck lift.

Best for

Loose skin or neck bands; see neck lift.

How Dr. İlker approaches chin liposuction

Op. Dr. Bilgehan İlker is a specialist in plastic, reconstructive and aesthetic surgery, trained at Dokuz Eylül University. Her own pages describe liposuction with fine cannulas as the effective way to remove fat under the chin, under local anaesthesia or sedation, with patients usually going home the same day.

She is just as clear about its limit. In younger patients liposuction alone is often enough, because the skin can still tighten by itself; later in life a skin-tightening step may be added under general anaesthesia. What decides it is the elasticity of your skin, checked in person.

  • Skin quality first Fat volume, skin quality, neck bands and chin shape are assessed before a plan is suggested.
  • Scars planned in advance Every operation leaves a scar. If skin is tightened, the scar sits under the chin, or near the ear when combined with a neck lift.
  • Real risks, plainly stated Irregularities and the small risk to the nerve of the lower lip are explained before you consent.

The operation, step by step

A typical day for liposuction alone; adding a lift makes the operation longer.

  1. Consultation and marking

Your goals, health, medicines, alcohol and tobacco use are reviewed, and your face and neck are measured, marked and photographed.

  2. Anaesthesia

Local anaesthesia, with sedation if needed; general anaesthesia if skin tightening is added.

  3. Small openings

A few openings of a few millimetres are made, typically under the chin and sometimes near the earlobes.

  4. Removing the fat

After tumescent fluid is injected, fat is removed in even layers with a fine cannula, avoiding the nerve that moves the lower lip.

  5. Closing and support

Small stitches or skin glue close the openings and a supportive chin strap goes on. Most people go home the same day.

Your days in Istanbul

ASPS advises waiting 7–10 days after facial procedures before flying, so plan for an examination before surgery and a check before you leave.

  1. Arrive in time to be examined

    Before surgery

UK guidance (BAAPS) suggests being examined in person at least 48 hours before surgery, or arriving 72 hours ahead if you have general anaesthesia.

  2. Home with a chin strap

    Surgery day

Keep your head raised and have someone with you overnight.

  3. Gentle and covered

    The first days

Wear the strap, move your neck gently and stay out of the sun. Skip alcohol, swimming and long tours.

  4. Stitches out and a final check

    Before you fly home

Stitches that do not dissolve usually come out after about a week. BAAPS recommends a check by your operating surgeon before you leave.

Recovery, day by day

Swelling and bruising usually ease within two weeks, while the contour keeps refining for months. These are typical ranges, not promises.

  1. Day 1

    Swollen and tight

Wear the chin strap, keep your head above heart level and do not put ice on the area.

  2. Days 2–3

    Move carefully

Avoid sudden head turns and clothes that pull over your head.

  3. Week 1

    Stitches out

Non-dissolving stitches are usually removed and discomfort is easing. Stay out of the sun.

  4. Week 2

    Back to work

Swelling has clearly reduced and most people are working again. Flying is usually possible after 7–10 days, once your surgeon agrees.

  5. Weeks 3–4

    Light exercise

Light exercise can usually restart. Some numbness may linger for several weeks.

  6. Months 2–3

    A sharper jawline

Residual swelling fades. Keep your weight stable.

  7. Month 6

    Settled

The area can take up to six months to settle fully; any irregularity is assessed at this stage.

  8. Month 12

    Scars maturing

If skin was tightened, protect the scar from the sun for the first year.

  • Bleeding, discharge, redness or swelling at an incision
  • Swelling under the chin that grows quickly, which may be a haematoma
  • A temperature above 38 °C (100.5 °F), or pain your medicines do not control
  • Chest pain, dizziness, or nausea and vomiting

In an emergency, call your local emergency number.

Aftercare at home

  • Wear the chin strap and dressings exactly as instructed.
  • Rest with your head above heart level.
  • Do not put ice directly on the treated area.
  • Avoid sudden head turns and clothes that go over your head at first.
  • Stay out of the sun while the skin heals.

Risks and how they are reduced

In a series of 435 patients treated with ultrasound-assisted liposuction under local anaesthesia, 3.45% had a complication. Reviews also describe rarer, more serious problems.

Choosing liposuction only for elastic skin, fine cannulas, even fat removal that keeps clear of the lower-lip nerve, a well-fitted chin strap and stopping nicotine all reduce these risks.

Risks and how they are reduced
RiskHow often

Swelling, bruising and numbness

Expected after surgery; numbness can last several weeks.

Common

Irregular contour or asymmetry

Uneven or dimpled areas, or a hollow under the chin. Fibrosis and asymmetry affected 0.69% in the large series.

Uncommon

Lower-lip weakness

The nerve that moves the lower lip lies close by. It was affected in 0.91% in the large series, and all recovered.

Uncommon

Loose skin

Skin that is not elastic enough can sag more after fat removal, which is why skin quality is checked first.

Varies

Skin darkening or sensitivity

Reported in 0.46% of patients in the large series.

Uncommon

Haematoma, infection or thick scars

A quickly growing swelling needs prompt attention. Very rarely, a serious deep neck infection has been reported.

Rare

Results and how long they last

The change shows as the swelling fades, and the jawline keeps sharpening for several months; full settling can take up to six months.

Fat cells that are removed do not come back, but those that remain can grow if you gain weight, and the skin keeps ageing.

How long it lasts: Long-lasting, provided your weight stays stable.

With any surgical or interventional procedure, results vary from person to person. We recommend a detailed consultation with your doctor before any procedure.

Questions to ask at your consultation

Take these to any surgeon you see — including us.

  1. Are you a specialist in plastic, reconstructive and aesthetic surgery, and where did you train?
  2. Is my skin elastic enough for liposuction alone, or do I need a chin tuck or neck lift?
  3. Will I be awake, sedated or under general anaesthesia, and where will the operation take place?
  4. How long should I wear the chin strap?
  5. How do you protect the nerve to the lower lip?
  6. Whom do I contact if a problem appears after I fly home?

What shapes your plan and quote

After Op. Dr. Bilgehan İlker has examined you, you receive a personal written plan that states what is included. It depends on:

  • Liposuction alone, or with a chin tuck, neck lift or chin surgery
  • Local anaesthesia, sedation or general anaesthesia
  • Where the operation takes place
  • Chin strap, medicines and check-ups during your stay

We do not publish prices: every plan is personal and is written for you after your assessment.

Frequently asked questions

Are chin liposuction results permanent?

Removed fat cells do not come back, so the result is long-lasting. The fat cells that remain can still enlarge if you gain weight, and skin keeps ageing, so a stable weight and a healthy routine help you keep the new jawline.

How long is chin liposuction recovery?

Swelling, bruising and tightness usually ease noticeably within about two weeks, and most people are back at work within one to two weeks. Light exercise usually restarts around weeks three to four, and the contour keeps refining for up to six months.

How long do I need to wear a chin strap?

The strap supports the skin while it settles under the chin. Authoritative sources give no single duration, so how long you wear it, and whether day and night, follows your surgeon's protocol. Ask for clear written instructions before you leave Istanbul.

Chin liposuction or neck lift: which do I need?

If your skin is elastic, liposuction may be enough. Clearly loose skin or visible neck bands call for a neck lift or chin tuck instead, because liposuction alone could leave the skin looking looser. An examination settles which applies to you.

Will my skin sag after chin liposuction?

It can if the skin is lax, which is why elasticity is checked first. In a series of 435 selected patients, loose skin after surgery was reported in only 0.23%. Careful selection, not technique alone, is what keeps this risk low.

Can chin liposuction affect my lower lip?

Rarely. The nerve that moves the lower lip runs close to the treated area. In a series of 435 patients it was affected in 0.91%, and all of them recovered. Tell your surgeon promptly if your smile looks uneven after surgery.

Is chin liposuction painful?

It is done under local anaesthesia, sometimes with sedation, so the area is numb during surgery. In one series under local anaesthesia, about two-thirds of patients reported mild pain afterwards and 6% reported severe pain. Ask how your pain will be managed.

Is there a non-surgical alternative to chin liposuction?

Fat-dissolving injections of deoxycholic acid can be an option when there is not much fat and the skin is not loose. They break down fat cells without incisions or stitches. With more fat, or looser skin, they are less suitable than surgery.

Will chin liposuction leave scars?

Liposuction alone leaves tiny marks a few millimetres long where the cannula entered, and these usually fade with time. If skin is tightened, the scar sits in the crease under the chin, or near the ears when combined with a neck lift.

How long should I stay in Istanbul for chin liposuction?

Plan on around 7 days for liposuction under local anaesthesia, and 7–10 days if skin is tightened under general anaesthesia. ASPS advises waiting 7–10 days after facial procedures before flying, and BAAPS recommends a check by your surgeon before you leave.
