Explicit consent text
Our enquiry forms ask four separate questions. This page shows their exact wording (version 2026-10-07) and explains what each one means. Only one of them asks for consent to use your health information, and you are free to say no.

1. Privacy notice: confirming that you have been informed
I have read and understood the privacy notice.
Form wording, required
This box confirms that we gave you our privacy notice before you sent us anything. It is not consent. Türkiye's Personal Data Protection Board requires the information notice and any request for consent to be presented separately, so we ask you only to confirm that you have read and understood the notice. The same information, in the format required by Turkish law, is in our KVKK information notice.
2. Explicit consent: your health information
This is the wording shown in the form, word for word:
Explicit consent — my health information. I give my explicit consent for Dr. Bilgehan İlker (Uzm. Dr. Bilgehan İlker Muayenehanesi, data controller) to process the health information I provide in this form: the procedure I am interested in and anything I write about my health. The purposes are to assess my enquiry, to contact me and to prepare a personalised treatment plan. For these purposes only, the information may be shared with the medical team, who are bound by professional secrecy, and, if treatment is planned, with the licensed hospital where it would take place. I can withdraw my consent at any time. Withdrawal does not affect processing carried out before it.
Form wording, answer required: 'I consent' or 'I do not consent'
Neither answer is selected in advance, and you choose one before you send the form. The form does not ask for photos.
If you choose 'I consent'
- You can tell us which procedure interests you and write about your health in the message box.
- We use this information only to assess your enquiry, to contact you and to prepare a personal treatment plan.
- It may be shared only with the medical team, who are bound by professional secrecy, and, if treatment is planned, with the licensed hospital where it would take place: [hospital name to be confirmed].
- We record the exact wording you agreed to, its version, and the date and time.
If you choose 'I do not consent'
- The procedure and message fields are switched off, and our server discards them even if they arrive.
- We receive only your name, country and contact details, and we will call or write to you.
- Saying no does not stop you from contacting us or arranging an examination.
Withdrawing your consent
You can withdraw your consent at any time, as easily as you gave it. Tell us by phone or WhatsApp on +90 542 280 27 51, by post to the address in our legal notice, or by e-mail at [e-mail address to be added]. We then stop using your health information for these purposes and delete it, unless the law requires us to keep it. Withdrawal does not affect processing carried out before it.
3. Updates: optional
I would like to receive information about treatments and news by email, SMS or WhatsApp. I can unsubscribe at any time, free of charge.
Form wording, optional
This box is not ticked in advance, and you do not need to tick it to get a reply. If you leave it empty, we contact you only about your enquiry. If you tick it, you can unsubscribe at any time, free of charge, by telling us through any of our contact details.
4. Age: required
I am 18 or older. If I am enquiring for someone under 18, I am their parent or legal guardian.
Form wording, required
This website is for adults. Treatment of anyone under 18 always needs the consent of a parent or legal guardian, so an enquiry about a child must come from them.
What we keep as a record
When you send a form, we store the following with your enquiry:
- which boxes you ticked, and your answer to the health-information question;
- the version of the consent wording, currently 2026-10-07;
- the exact health-consent text you agreed to, if you chose 'I consent';
- the date and time, and the page where you sent the form.
This record lets us show what you agreed to, and when. Our privacy notice explains how long we keep it.

WhatsApp is run by Meta. Messages you send there are processed on Meta's servers under its own terms.
