Chin surgery (genioplasty) in Istanbul
Chin surgery changes how far the chin projects, or its length or symmetry, to balance the profile. Here you can compare implants with moving the bone, see how recovery goes and consider whether filler might be enough.
- Operation time Under 1 hour to about 3 hours
- Hospital stay Usually same day; sometimes one night
- Time in Istanbul About 7–10 days after surgery
- Back to work About 1 week (genioplasty 7–10 days)
What is chin surgery?
Chin surgery changes the projection, length or symmetry of the chin to balance it with the nose, lips and jaw. There are two main methods: a chin implant, usually silicone or porous polyethylene, placed on the bone; and sliding genioplasty, in which the bone is cut, moved and fixed with plates and screws. A prominent chin can be reduced by reshaping the bone. Surgery is done through the mouth or a small cut under the chin, takes from under an hour to about 3 hours, and most people return to work after about a week.
What it can do
- Bring a receding or small chin forward
- Reduce a chin that projects too far
- Lengthen, shorten or straighten an uneven chin by moving the bone
What it cannot do
- Correct a bite problem, which needs jaw surgery
- Be judged early: swelling takes about 6 weeks to settle
Is it right for you?
Chin surgery suits adults whose chin is out of balance with the face and whose bite is normal. Suitability is confirmed in person, sometimes with X-rays.
Usually a good fit if
- Your chin looks small, short or set back in profile
- Your chin projects too far, or is too long, too short or uneven
- Your teeth meet normally when you bite
- Your face has finished growing, your weight is stable and you can stop nicotine 6 weeks before surgery
Better to wait or consider alternatives if
- Your teeth do not meet properly: the jaw itself may need orthognathic surgery with a maxillofacial surgeon
- For an implant: your chin is short in height, or you have reacted to implant material before
- You have an infection in your mouth, teeth or gums
- You expect a new chin to change how you feel about your whole face
Techniques
The choice depends on how much change you need, in which direction, and on your bite. Porous polyethylene implants erode bone less than silicone but are harder to remove.
Chin implant through the mouth
A pocket is made through an incision inside the lower lip and the implant is placed on the bone. There is no outside scar; whether infection is more likely than with an under-chin incision is debated.
Best for
Mild to moderate chin retrusion
Chin implant through a small cut under the chin
The incision sits in the natural crease under the chin, leaving a small scar; one study found fewer wound openings than with the mouth approach.
Best for
Mild to moderate retrusion without a mouth incision
Sliding genioplasty
Through the mouth, the chin bone is cut, moved forward, back or sideways (shortened or lengthened if needed) and fixed with plates and screws.
Best for
Asymmetry, long or short chins and larger changes
Chin reduction
Excess bone is removed and reshaped, usually under general anaesthesia as a day case.
Best for
A chin that projects too far

How Dr. İlker approaches chin surgery
Op. Dr. Bilgehan İlker is a specialist in plastic, reconstructive and aesthetic surgery, trained at Dokuz Eylül University. Her own chin pages describe jawline filler, the non-surgical option; this page sets out the surgical methods from the medical literature so you can weigh both.
Every plan starts with an in-person examination of your profile and bite, with imaging if needed.
- Profile assessed as a whole Chin size changes how large the nose looks, so the two are assessed together and often planned with rhinoplasty.
- Bite checked first If your teeth do not meet properly, chin surgery alone is not the answer, and a maxillofacial (jaw) surgeon's opinion is advised.
- Filler or surgery, compared honestly For mild volume loss, filler may be enough, though its effect is temporary. A structural change needs surgery.
The operation, step by step
A typical sequence:
-
Assessment
Your profile, lip-chin angle and bite are analysed, with X-rays if needed.
-
Anaesthesia
Local, sedation or general for an implant; usually general anaesthesia for genioplasty.
-
Incision
Inside the mouth, in the groove between the lower lip and gum, or in the crease under the chin.
-
Implant or bone work
An implant goes into a snug pocket on the bone, protecting the nerve to the lower lip; in genioplasty the bone is cut, moved and fixed.
-
Closing and dressing
The wound is closed in layers, and tape or a supportive dressing stays on for 2–4 days.
Your days in Istanbul
Plan for an examination before surgery and a check before you fly.
-
Examination and planning
Before surgery
BAAPS advises a face-to-face examination at least 48 hours before surgery, and arriving at least 72 hours before general anaesthesia if you travel far.
-
Soft food and rest
The first days
Expect swelling and a tight feeling. Eat soft food, rinse your mouth after meals and rest with your head raised.
-
Cleared to fly
Before you fly home
ASPS advises waiting 7–10 days after facial surgery before flying.
Recovery, day by day
Typical timings, not promises.
- Day 1
Tight and sore
Expect moderate pain for two days and soreness at any stitches in the mouth. Keep your head raised and eat liquids or very soft food.
- Days 2–3
Swelling and numbness
Swelling, bruising and temporary numbness of the lower lip are common. The dressing comes off at days 2–4; rinse your mouth after meals.
- Week 1
Tightness easing
The tight feeling usually eases. Avoid heavy lifting and strenuous activity; many people with an implant return to work.
- Week 2
Back to routine
Swelling and bruising can last 2–3 weeks. Most people return to work 7–10 days after genioplasty; flying is usually fine after 7–10 days.
- Weeks 3–4
Normal food
Swelling falls noticeably. Return to normal food and exercise as your surgeon advises.
- Months 2–3
Settled shape
Most swelling has gone by about week 6.
- Month 6
Sensation
Any change in feeling is usually temporary but can last months; report numbness that persists.
- Month 12
Long-term
The result is long-lasting. Protect any scar under the chin from the sun in the first year.

Aftercare at home
- Sleep with your head raised and use cold compresses.
- Eat liquid or soft food, and rinse with salt water or the prescribed mouthwash before and after meals.
- Brush gently, protecting any incision inside the mouth.
- Do not press on or try to move the implant.
- Avoid heavy lifting, bending and strenuous activity for at least a week.
Risks and how they are reduced
Implants are generally low-risk; figures come from published studies, not from this clinic.
Checking the bite and teeth first, choosing a method that fits your anatomy, protecting the lip nerve, careful closure and stopping nicotine all reduce risk.
| Risk | How often |
|---|---|
Numbness of the lower lip or chinTemporary numbness is common after both methods and usually recovers; lasting numbness is rare. Implant reviews report nerve injury in about 2.4%. | Common |
Implant shifting or bone erosionImplants were misplaced in 2.5% and later shifted in 5.0%; the bone beneath eroded in 8.3%. Edges can sometimes be felt. | Uncommon |
InfectionFrom under 1% to about 5% with implants; a persistent infection means removing the implant. One comparison found more infections with implants than with genioplasty. | Varies |
Genioplasty problemsOne series of 59 patients reported infection in 3.4%, haematoma in 8.5% and temporary numbness in 6.8%. | Uncommon |
Shape problemsAsymmetry, a sagging 'witch's chin' after inadequate closure, or a deeper hollow in front of the jowls. | Uncommon |
General surgical risksBleeding, fluid collection, lasting pain, revision and blood clots can follow any operation. | Uncommon |
Results and how long they last
Most swelling settles by about 6 weeks. Solid implants and repositioned bone give a long-lasting change, though an implant can shift or erode the bone over the years. Some people later fine-tune the result with filler. Results vary from person to person.
How long it lasts: Long-lasting, though implants can shift or erode the bone over time.
With any surgical or interventional procedure, results vary from person to person. We recommend a detailed consultation with your doctor before any procedure.

Before and afterQuestions to ask at your consultation
Take these to any surgeon you see — including us.
- What is your specialist training, and how often do you perform chin surgery?
- Would an implant or a sliding genioplasty suit my chin, and why?
- Is my bite normal, or do I need a jaw surgeon's opinion?
- Which implant material do you use, and can it be removed?
- Will the incision be inside my mouth or under my chin?
- Whom do I contact after I fly home if there is a problem?
What shapes your plan and quote
We do not publish prices. After Op. Dr. Bilgehan İlker has assessed your case, you receive a personal written plan. It depends on:
- An implant, a sliding genioplasty or a reduction
- The implant material, if one is used
- The type of anaesthesia and hospital stay
- Any combined procedure, such as rhinoplasty
We do not publish prices: every plan is personal and is written for you after your assessment.
Frequently asked questions
Chin implant or genioplasty: which is better?
It depends on your chin. An implant can be enough for mild to moderate retrusion, while moving the bone suits asymmetry, a chin that is too long or short, or larger changes. One comparison found fewer infections and higher satisfaction with genioplasty, though it was not randomised.
Is a chin implant permanent?
Solid implants give a long-lasting result, but not an unchangeable one: over the years the bone beneath can erode (8.3% in one review) or the implant can shift (5%). An implant can also be removed in a further operation if needed.
Can a chin implant be removed?
Yes, with a further operation. Silicone implants are easier to remove than porous polyethylene ones, because tissue grows into the porous material. If an implant becomes infected and the infection persists, it is removed, and a new one can be placed later.
How long is recovery after chin surgery?
Most people return to work after about a week with an implant, or 7–10 days after genioplasty. Swelling and bruising can last 2–3 weeks, and most swelling has settled by about 6 weeks. Strenuous exercise waits for a few weeks.
Is chin surgery painful?
Expect moderate pain for the first two days, plus soreness at the stitches if the incision is inside your mouth. A tight feeling usually eases within a week. Pain that keeps increasing, or swelling with redness and discharge, should be checked promptly.
Can you feel a chin implant?
Sometimes. The edges of an implant can occasionally be felt or seen under the skin, which is one reason its size and position are planned carefully. While it heals, avoid pressing on or moving it, and report anything that seems to shift.
Will chin surgery leave a scar?
Not a visible one if the incision is inside the mouth, which is also the route for genioplasty. An incision under the chin leaves a small scar in the natural crease, which fades over time; protect it from the sun for the first year.
Will my lip go numb?
Temporary numbness of the lower lip or chin is common after both implants and genioplasty, because the nerve that supplies the lip runs close by. It usually recovers, sometimes over months. Lasting numbness is rare and should be reported if it does not improve.
Is filler an alternative to chin surgery?
For mild volume loss, it can be. Hyaluronic acid filler is non-surgical and temporary, while surgery gives a long-lasting structural change, so filler suits smaller changes. Some people try jawline filler first; others use filler later to fine-tune a surgical result.
How long should I stay in Istanbul for chin surgery?
Plan on about 7 days after a short operation under local anaesthesia, or 7–10 days after an implant or genioplasty under general anaesthesia, so the dressing can come off and you can be checked before you leave. ASPS advises waiting 7–10 days after facial surgery before flying.
