Gynaecomastia: the most common question

Enlarged breast tissue in men is common. It affects up to half of adolescent boys, usually settling on its own within six months to two years, and between a quarter and two thirds of men aged 50 to 80. According to ISAPS, the International Society of Aesthetic Plastic Surgery, gynaecomastia surgery was the second most common cosmetic operation in men worldwide in 2024.

True gynaecomastia is an increase in breast gland tissue; when the fullness is fat only, it is called pseudogynaecomastia. Liposuction removes fat but not gland, so gynaecomastia surgery often combines liposuction with removal of tissue through a small incision at the edge of the areola. Because some medicines, anabolic steroids and hormonal conditions can cause it, the cause is looked at first, and a one-sided or hard lump is always investigated.

Face surgery for men

Rhinoplasty, or septorhinoplasty when breathing is also a concern, eyelid surgery, ear correction, chin surgery and chin liposuction are among the face procedures men ask about. A few things are planned differently for men:

  • Facelift scars are harder to hide with short hair, and after a facelift the beard line sits closer to the ear. Incision placement is discussed at your consultation.
  • Men have a higher risk of bleeding under the skin (haematoma) after a facelift. Tight blood-pressure control lowers it: in one series, from 8.7% to 3.97% in men.
  • After upper eyelid surgery, a low brow can become more noticeable, especially in men, so the brow position is checked before eyelid surgery is planned.

Body contouring for men

Liposuction of the abdomen, flanks and chest is common in men. HD liposuction aims to show the outline of the muscles and suits men who are already lean, exercise regularly and keep a stable weight; overdone definition can look unnatural. After major weight loss, an arm lift or body lift removes loose skin, and scar revision can improve how an old scar looks.

Non-surgical options

Anti-wrinkle injections soften frown and forehead lines, and jawline filler can add definition to the jaw and chin. The effects are temporary: usually 3–4 months for anti-wrinkle injections, and up to about 12 months for jawline filler, depending on the product.

Frequently asked questions

Can gynaecomastia come back after surgery?

In most cases the result lasts. Gynaecomastia can return, however, if its cause continues: anabolic steroids, some prescription medicines, or significant weight gain. Avoiding these where you can and keeping your weight stable protects the result. If a prescribed medicine may be involved, discuss alternatives with the doctor who prescribed it rather than stopping it yourself.

When can I go back to the gym after gynaecomastia surgery?

Gradually. NHS guidance is to avoid stretching, strenuous exercise and heavy lifting for about three weeks, and most men return to full activity at around six weeks. Dr. İlker's own advice, on her Turkish website, is to wait until after the sixth week before returning to sport, swimming and the sea, and to protect the chest from knocks.

Can gynaecomastia surgery be done under local anaesthetic?

Sometimes. It is usually done under general anaesthetic, and sometimes under local anaesthetic with sedation. On her Turkish website, Dr. İlker describes using local anaesthetic for milder cases and general anaesthetic for more extensive ones. The operation typically takes about one and a half to two hours, and you may go home the same day or stay one night.

How much time off work should I plan?

It depends on the procedure and on your job. After gynaecomastia surgery, plan for a few days to about two weeks; after rhinoplasty or eyelid surgery, one to two weeks; and after liposuction, from a few days to around two weeks for desk work. Physical work and heavy lifting need longer. If you are travelling, add the time you need to stay in Istanbul.
